How To Register Your Child

Submit child’s documents via email at admin@hoklearning.net
Documents such as:
- Physical examination form
- Dental examination
- Birth Certificate
- Proof of income for last 30 days
- Proof of medical insurance for a child

Submit your $100.00 Registration Fee to our Zelle Account Number 773-344-8255
03.
Pick up the enrollment package from the physical center or call to have the enrollment package mailed to you.

To pre-enroll your child, Please click here,

Please submit all required documentation via email to admin@hoklearning.net or bring copies to your appointment.
